Staff Reports are records made on a member's account by staff. They can acknowledge commendable behavior or document disciplinary incidents.

Staff Reports are the formal record-keeping system Lighthouse uses to document notable incidents and actions involving members. When staff observe behavior that warrants acknowledgment — positive or negative — they create a staff report as the official record.
A staff report is a record filed by a staff member about a specific incident or pattern of behavior involving a member of the community. Reports capture what happened, where it happened, who was involved, and what action (if any) was taken.
Reports are attached to a member's account and are part of how staff assess a member's history when making moderation decisions.
Reports are NOT disciplinary or punishment. Receiving a report simply means the staff has noted something that required some sort of staff interaction.
Staff members even write reports on themselves, such as when FinalAsgard shared an image that included a curse word camouflaged into the image. It required a staff member to delete the photo, which is a staff interaction, therefore requiring a Trivial report to be made.
Not all staff reports are disciplinary. Reports are classified by severity:

| Commendation | Recognizes positive behavior — helpfulness, exceptional conduct, going above and beyond for the community. |
| Trivial | Minor or technical violations that are typically handled on the spot with a quick reminder. |
| Minor | A clear rule violation that required staff intervention but is not serious. |
| Moderate | A meaningful violation that may affect other members or repeated minor behavior. |
| Major | Serious violations that may warrant Brig placement or further action. |
| Severe | The most serious category — reserved for significant harm, safety concerns, or repeated serious violations. |
Commendations are a recognition of good community participation and carry no disciplinary weight. Disciplinary severity levels are used by staff to track a member's history over time.
Once published, the report is finalized and becomes part of the member's record.
Members can view their own published staff reports on the Lighthouse website. When a report is published, the member receives a notification.

Members do not see draft reports, the identity of the reporting staff member, or internal staff notes.
Parents of minor members can also view published reports on their child's account.

For published reports, staff and the member involved may open discussion topics to communicate about the incident. These discussions are visible to the relevant parties. Members can participate in discussions about their own reports.
Staff reports are permanent records. Once published, a report cannot be removed from a member's account. If a member believes a report is factually inaccurate, they should raise the concern through a support ticket addressed to Command.
Staff reports document incidents. The Brig is a separate action — it restricts a member's community access as a consequence of those incidents. A staff report does not automatically result in Brig placement; that is a separate decision made by Command or senior Quartermaster staff based on the full context.
